Quarter inch is the American classroom default, the ruling in the composition books and the loose-leaf pads sold as "graph paper" without further explanation in the US. Four squares to the inch, 6.35 mm a side, and — with the heavy line set to every fourth square, as it is here — one heavy block per inch.
The imperial arithmetic is what makes it worth having rather than substituting 5 mm. One square is a quarter inch, two are a half, and a block is an inch, so a measurement in inches and quarters reads straight off the sheet without conversion. That is exactly what quilting, woodworking layout and any US-dimensioned drawing wants.
Do not swap it for 5 mm paper on the grounds that they look alike. A quarter inch is 6.35 mm, twenty-seven percent more square area, and a pattern printed on the wrong one comes out the wrong size in a way you will not notice until it is cut.
The heavy line is set to every fourth square here rather than every fifth, which is the change that makes the sheet imperial rather than metric-with-inch-squares: one heavy block is one inch, so a dimension in inches and quarters reads straight off the page.

What is 4 squares per inch?
It is quarter-inch paper described the other way round. Four squares to an inch means each square is a quarter of an inch, or 6.35 mm — the same sheet.
Can I use 5 mm paper instead of quarter inch?
Only for rough work. A quarter inch is 6.35 mm, so anything you scale off the sheet will come out 27 percent smaller in area than intended. For a quilt block or a cutting layout that matters a great deal.
How do I check the squares printed at exactly a quarter inch?
Measure across four squares — one heavy block — which has to be exactly one inch. Measure across sixteen for a four-inch check if your ruler is long enough; the longer the run, the smaller the scaling error you can catch.
